Create a new query based on the Pet and PetType tables and add the following fields:
- From the Pet table: PetName, Gender, PetDOB, Email, SpecialRequirements, VetBills
- From the PetType table: PetType, Limbs, Picture
Save the query with the name PetDetails.
Create a new form based on your query using the AutoForm Columnar wizard. Modify the form so that is resembles the form below:

Feel free to experiment with the formatting and layout of the form
Save the form with the name Pet Details and close it.
