Exercise: Form showing worksheet names on open

1) Download the files needed to start this exercise: exercise-565.zip (and click here if you experience any problems unzipping this file after downloading it).
2) Open the file in the folder shown above.
3) Create a form/macro such that when you open the workbook you see a form allowing you to choose which worksheet to go to:

4) Try adding, renaming and deleting worksheets, then save and close your form.  When you open it up, is it still giving an up-to-date list?
