Exercise: Dating form including DateTimePicker and ComboBox

1)	Create a new form called frmApplicationForm which people can use to apply for membership of your dating agency:
	The formatting is completely up to you!
2)	Your form should include: A DateTimePicker control (apply the ToString method to the Value property to get the date chosen) A ComboBox control (retrieve the SelectedItem.Text property)
3)	Get the Apply now button to display a message like this:
	Use a MessageBox to display the user's choices
4)	Test that your form works!
