| 1) | Download the files needed to start this exercise: exercise-354.zip (and click here if you experience any problems unzipping this file after downloading it). |
| 2) | Create a new query based on the Pet and PetType tables and add the following fields:

| 3) | Save the query with the name PetDetails. |
| 4) | Create a new form based on your query using the AutoForm Columnar wizard. Modify the form so that is resembles the form below: |
